Patient readmissions have become revolving doors in healthcare.
One of the continuing problems with the quality of treatment services have been government concerns over the high numbers of patients failing to complete treatment. Many times after discharge, patients are re-admitted for care at the same or other facilities.
Even when patients are discharged from services as “treatment complete”, they are often left with no aftercare plan. Usually patients have infrequent referrals to basic After Care services.
CARF Accreditation surveyors frequently mention how treatment programs usually lack linkages with the community. Patients are left to find AA sponsors, networking for jobs, sober living arrangements, and communications with treatment case managers is lacking. Numerous treatment programs are cited on CARF surveys for their clinical programming failures. Frequently, these same programs lack basic data collection systems.
